What to eat on Mounjaro injection day: a nausea-friendly plan
Mounjaro (tirzepatide) can be injected at any time of day, with or without food, so the leaflet does not prescribe an injection-day menu. But nausea is very common, most people feel it most in the day or two after injecting, and what you eat around the dose makes a real difference to how those days go. This is a low-risk, practical plan: small, bland, protein-led, and light on fat. It does not replace advice from your prescriber or a dietitian.
Key facts
- The leaflet allows injection at any time, with or without meals.[1] There is no evidence that injecting on an empty or full stomach changes side effects.
- Nausea is very common in weight management and tends to be worst when starting and after each dose increase.[1]
- Because tirzepatide slows stomach emptying, large, fatty or very rich meals sit longer and are the most common trigger for feeling sick.[2]
- The NHS advice for feeling sick is small frequent meals, bland food, plenty of fluids in sips, and avoiding strong smells and lying down straight after eating.[3]
- Red flag: severe, persistent stomach pain with or without nausea and vomiting needs urgent help; see the pancreatitis guide.
The principles
- Small and often beats three normal meals. Half portions, five times a day.
- Protein at every eating occasion, so a low-appetite day does not become a no-protein day.
- Low fat, low grease. Fried food, creamy sauces, pastry and takeaways are the usual culprits.
- Bland and cool. Cold or room-temperature food has less smell, and smell is a nausea trigger.
- Fluids between meals, in sips, not a pint with dinner.
- Stay upright for an hour or two after eating.
- Stop at comfortably full. On tirzepatide the gap between full and too full is narrow.
An injection-day plan
| Time | Eat | Skip |
|---|---|---|
| Breakfast | Porridge made with milk, or toast with a poached egg, or yoghurt with banana | Fried breakfast, pastries |
| Mid-morning | A few plain crackers with a little cheese, or a small pot of yoghurt | Coffee on an empty stomach if it bothers you |
| Lunch | Chicken or lentil soup with bread, or a small jacket potato with cottage cheese | Creamy pasta, pizza, anything deep-fried |
| Afternoon | Apple slices, a rice cake with nut butter, or a boiled egg | Chocolate and sweets, which often sit badly |
| Evening (early) | Baked white fish or plain chicken with rice or potato and steamed vegetables | Curry, large steaks, rich sauces, alcohol |
| Before bed | Nothing, or a small glass of milk if hunger wakes you | Late large meals, which drive reflux |
Swap freely within the principles. Ginger tea, peppermint tea and plain water with a slice of lemon are the drinks most people find settle the stomach; fizzy drinks split the room, so try and see.
Foods that tend to sit well
Plain rice and potato, toast and crackers, porridge, bananas, apples and melon, yoghurt and cottage cheese, eggs, white fish, chicken breast, tofu, lentil or chicken soup, steamed vegetables, ginger in any form.
Foods that tend not to
Fried and greasy food, cream-based sauces, pastry, large red-meat portions, very spicy dishes, big raw salads on a bad day, fizzy sugary drinks, alcohol, and anything eaten too fast or too much of. Many people also find their tolerance of coffee drops in the first weeks.
If you vomit
Rest the stomach for an hour or so, then start with sips of water or diluted squash and move to bland food when you can. Our sick-day rules guide covers repeated vomiting and diarrhoea, and the electrolytes guide explains what to drink. If you cannot keep fluids down, contact NHS 111 or your prescriber; the leaflet notes that dehydration can affect the kidneys.[1]
Common questions
Should I inject Mounjaro before or after eating?
Either. The leaflet allows injection at any time of day with or without food, and there is no evidence that timing relative to meals changes side effects. Choose whatever is easiest to keep to each week.
What should I eat the day after my Mounjaro injection?
The same plan: small, bland, protein-led meals, fluids in sips between them, and nothing large or greasy. Many people feel the dose most on day two, so keep the plan going for two or three days.
Is it better to inject Mounjaro at night?
Some people inject in the evening so that the peak of nausea falls overnight and the next day. Others find it disturbs their sleep. There is no clinical rule; pick a time you can keep, and tell your prescriber if you want to change it.
Can I take anti-sickness tablets on injection day?
Only if your prescriber has prescribed or approved them for you. This page does not recommend taking unprescribed anti-sickness medicine. If nausea is bad enough to want tablets, that is worth telling your prescriber, who can also review your dose.
