Before you restart: have this ready
- Proof of your last prescription. An order confirmation, dispensing label or pharmacy email with the dose and date. Most restart-friendly pharmacies will not accept you below their joining BMI without it.
- Your dates. Last injection date and the dose. That single fact decides whether you keep your dose or drop to 2.5mg.
- Your starting weight and current weight. Pharmacies accepting a low maintenance BMI want to see that the weight loss came from treatment.
- Why you stopped. Cost, stock, holiday, illness, surgery or side effects. If it was side effects, expect a slower restart. If it was surgery or pregnancy, you need the prescriber to clear you first.
- Don’t double up. A restart at 2.5mg is not a failure; the first 4 weeks are about tolerance, not weight loss, and you can move up on schedule.
What happens to your weight when you stop and restart Mounjaro
Appetite comes back within two to three weeks of the last dose as tirzepatide clears, and most people regain some weight over a break of a month or more. In the SURMOUNT-4 trial, people switched to placebo after 36 weeks regained around two-thirds of the weight they had lost within a year, while those who stayed on treatment kept losing. Restarting reverses that, but the first month back at 2.5mg is usually weight-stable rather than a fast loss.
Plain English: a short break of under two weeks costs you very little. A break of a month or more usually means starting from 2.5mg again, some regained weight, and four to eight weeks before you are back at the dose that was working. The fix is to avoid gaps where you can: a 2 or 3 pen bundle at a cheaper pharmacy is often less than the cost of restarting from scratch.
Restarting after surgery, pregnancy or illness
Stop-and-restart around surgery is common: some surgical teams ask for Mounjaro to be paused for a week before a general anaesthetic because it slows stomach emptying, and current UK guidance varies, so follow your surgical team. Restart only when your surgical team says so, and treat it as a full restart if the gap is over four weeks. Mounjaro is not used in pregnancy or while breastfeeding; a restart afterwards is a new assessment, not a continuation.
Restarting Mounjaro: questions people ask
Can I stop Mounjaro for 2 weeks and restart on the same dose?
Usually yes. Two missed doses is within the window most UK pharmacies treat as a continuation, so you carry on at the dose you were on. Expect a little more nausea for the first day or two.
Restarting Mounjaro after a month: which dose?
Four weeks off is the point where nearly every UK pharmacy restarts you at 2.5mg and re-titrates. A 5mg restart after a month off is rare and only agreed by a prescriber on individual clinical grounds; plan on 2.5mg.
Restarting Mounjaro after 3 months or 6 months
A full restart: 2.5mg for four weeks, then step up. Longer breaks also mean a fresh eligibility check, so if your BMI has fallen below 27 you need a pharmacy with a low maintenance BMI (see the list above) and proof of your previous treatment.
Can you restart Mounjaro on 5mg?
Within a short break (under about four weeks) some pharmacies keep you on 5mg or step you down to it; Get A Drip, for example, restarts at 5mg after exactly three missed doses. After a month or more off, a 5mg restart is rare and down to the individual prescriber’s clinical judgement. No pharmacy on this page restarts above 5mg after a month off.
How long can you be off Mounjaro before restarting from scratch?

Yes, at a restart-friendly pharmacy. Maintenance BMI thresholds on this page run from 19 up to 23, and every pharmacy will want evidence that the weight came off on treatment. Use the Joining BMI filter to narrow the list, then choose on service and price, not on the lowest number.
Does stopping and restarting Mounjaro affect weight loss?
Breaks of a month or more usually mean some weight regain and four to eight weeks of re-titration before you are back at an effective dose. Short breaks of under two weeks have little effect.
Do I need a new consultation to restart?
At the same pharmacy, a short review. At a new pharmacy, a full online assessment plus proof of prior treatment. Neither should cost extra beyond the pen at the pharmacies listed here; consultation is included in the price.
