Fifth Dose Mounjaro: Why Pens Hold Four Doses and How to Avoid a Gap
People search for a “fifth dose” of Mounjaro for two different reasons: some mean the leftover liquid in the old pen, and some have simply run out before the next pen arrives. This page deals with the second problem, which has become more common since the 2026 pen redesign removed the leftover. - Most online pharmacies dispatch in one to three working days and many do not process orders over the weekend, so the reliable habit is to reorder on the day of your third injection, not your fourth.
- Pharmacies vary in delivery speed, cut-off times and whether they let you buy a single pen at short notice. These are worth comparing before you need them.
Why the pen holds exactly four doses
Each KwikPen is licensed to deliver four doses of a single strength, one per week, which matches the four-week minimum between dose increases. Everything about the pen, from the plunger travel to the leaflet instructions, is tested against those four doses. Older pens had extra liquid inside to allow for priming, and some people managed to inject part of it as a fifth week. Eli Lilly has now changed the pen so that after the fourth dose the plunger sits at the end of the cartridge. If your last few pens have looked different from earlier ones, that is why.
What the leaflet says about a late or missed dose
The Patient Information Leaflet is clear on timing:
- Within four days of your usual day: take the dose as soon as you can, then go back to your normal weekly day.
- More than four days late: skip that dose. Take the next one on your usual day. Do not double up.
- Changing your injection day: you can move it, as long as at least three days have passed since the last dose.
Tirzepatide has a half-life of about five days, so a single missed week leaves some medicine in your system and most people notice only a modest return of appetite. After two or more missed weeks the medicine has largely cleared, and restarting at your previous dose can bring back the nausea you had at the start. Timing your reorder
The gap between pens is almost always a logistics problem rather than a medical one. A few habits remove it:
- Order at dose three. That gives a full week for prescriber approval, dispensing and delivery, with the weekend accounted for.
- Know your pharmacy’s cut-off. Many process orders placed before early afternoon for same-day dispatch and hold later ones until the next working day. Friday afternoon orders often move on Monday.
- Check whether a repeat needs a new consultation. Some providers require a check-in form every order, others every few months. A form left unanswered is a common cause of delay.
- Watch the dose step. If you are moving up a strength, the new pen is a different product and the prescriber has to approve the increase. Build that into your timing.
- Keep the pen in the fridge until you need it. A pen ordered early keeps until its expiry date if refrigerated, and for up to 30 days at room temperature once in use.
If a delivery is late
First, contact the pharmacy and ask for the tracking number and the dispatch date; that tells you whether the problem is with them or with the courier. Use the four-day window in the leaflet rather than assuming the week is lost. If the pen will clearly not arrive in time, some pharmacies can sell a single pen for collection or next-day delivery, and it is legitimate to buy one pen from a second GPhC-registered pharmacy while keeping your usual provider, provided you tell both prescribers what you are taking. Never buy from a seller that does not hold a consultation with you first, and never from social media; the MHRA has issued repeated warnings about counterfeit pens sold that way.

Common questions
Can I take my dose a day or two late?
Yes. The leaflet allows a dose up to four days after your usual day. Take it, then return to your normal schedule.
I missed a week. Should I take two doses to catch up?
No. Never take two doses within three days of each other. Skip the missed dose and take the next one on your usual day.
Can I split a higher-strength pen to stretch it?
The pen is fixed-dose and the licence does not allow it to be dialled to a different strength. Doing so gives an unmeasured dose and is not something any prescriber will support.
Can I order a pen from a second pharmacy if my usual one is late?
You can, as long as the second pharmacy is GPhC-registered and carries out its own consultation, and you tell both prescribers. Do not run two supplies at once.
