NHS vs Private Mounjaro: Cost, Wait and Care Compared
On the NHS you pay £9.90 per prescription item in England, or nothing if you are exempt, but you have to fit a priority cohort and wait your turn. Privately you start once a prescriber has checked you, and you pay the full price every month. This page sets the two side by side so you can see which one is open to you.
Key facts
- NHS Mounjaro costs £9.90 per item in England, or nothing with an exemption. A 12-month prescription prepayment certificate costs £114.50.
- Around 90 to 95% of UK Mounjaro users pay privately.
- NHS access runs by cohort. Full rollout is planned to take up to 12 years.
- Private treatment starts after an online consultation with identity and weight checks, usually within days.
- The medicine is identical on both routes. Mounjaro is made by Eli Lilly and there is no NHS version and no private version.
The two routes side by side
| Factor | NHS in England | Private |
|---|---|---|
| Who qualifies | Priority cohorts only. BMI 40 or more with at least 4 qualifying conditions, then BMI 35 to 39.9 with 4 | The prescriber decides within the licence and the pharmacy's own policy |
| How long you wait | Depends on your cohort and whether your practice prescribes yet. Rollout runs to 2037 or beyond | Days. Treatment starts once checks are complete |
| Monthly cost | £9.90 per item, or free if exempt | The full price, which changes by dose and pharmacy |
| Who prescribes | Your GP practice, if it has opted in | A prescriber at a registered online pharmacy or clinic |
| Dose changes | Managed in routine reviews | You order the next strength when you step up |
| If you stop | You rejoin the pathway under the rules at the time | You can restart whenever a prescriber agrees |
For most people this is not a choice. If you are not in an open cohort, the NHS route is not available to you yet, whatever your BMI.
What each route really costs over a year
The NHS figure is easy. In England you pay the standard prescription charge per item, £9.90, unless you are exempt. If you collect several items a month, a prescription prepayment certificate caps the year at £114.50.
The private figure is the one people underestimate, because the first month is almost never the real price. Most pharmacies sell the first pen below cost to win you, then move you to a repeat price that is higher. Work out the cost of the dose you expect to settle on, not the dose you start on.
Check the real number before you decide. Our discount price list shows first-order totals across every verified pharmacy, and the retail price list shows what the same pharmacies charge from month two. The gap between the two is the number that decides your year.
The care is different, and not always in the direction you expect
NHS treatment sits inside your medical record. Your GP sees your history, your other medicines and your test results, and the prescription is part of that picture.
Private treatment is faster and more flexible, and good pharmacies run proper checks. The weakness is that your prescriber usually cannot see your NHS record and relies on what you tell them. Tell them everything, and tell your GP you are taking it.
One thing people get wrong: paying privately does not disqualify you from NHS treatment later. It also does not move you up the queue. You are assessed against the cohort rules that apply at the time.
Which route applies to you
- Check whether you are in an open cohort. The NHS Mounjaro guide has an eligibility checker that answers this in under a minute.
- If you are, go to your GP practice. Our step-by-step guide covers the conversation and what happens next.
- If you are not, and you still want treatment, the private route is your only option today. Compare the total checkout price for your dose rather than the advertised headline.
- If you can wait, you can do both in sequence. Start privately now and move to the NHS when your cohort opens, as long as you meet the rules then.
Common questions
Is NHS Mounjaro the same medicine as private Mounjaro?
Yes. Same manufacturer, same pens, same doses, same licence. The only difference is who prescribes it and what you pay.
Can I switch from private to NHS?
Yes, if you meet the cohort rules in force when you apply. Being on it privately neither disqualifies you nor speeds you up.
Will my GP prescribe it if I ask?
Only if the practice has opted into the scheme and you meet the criteria of a cohort that is currently open. A GP cannot prescribe outside the rollout rules just because you ask.
Is private treatment safe?
It is, when the pharmacy is registered with the General Pharmaceutical Council and runs a proper assessment. Check the registration before ordering, and be honest in the consultation, because your prescriber cannot see your NHS record.
Does paying privately cost the NHS anything?
No. Private prescriptions are paid for in full by you.
