NHS Mounjaro Rollout: Who Gets It Each Year

The NHS is releasing Mounjaro one group at a time, starting with the people in greatest clinical need. Two groups are open now. This page tracks which cohort is open, which is next, and what has been announced rather than rumoured.

Key facts

  • The rollout began on 23 June 2025 in GP practices in England.
  • Cohort I opened first: BMI 40 or more with at least 4 of 5 qualifying conditions.
  • Cohort II opened on 23 June 2026: BMI 35 to 39.9 with at least 4 conditions.
  • Cohort III is due from 2027/28: BMI 40 or more with 3 conditions.
  • NHS England plans for everyone eligible under NICE TA1026 to have access within a maximum of 12 years.

Where the rollout stands today

Last checked 27 October 2026. Cohorts I and II are open in primary care. We have found no NHS England, DHSC or NICE announcement offering weight loss injections on the NHS to people in the overweight BMI range. The next dated milestone is the NICE decision on Foundayo (orforglipron), expected on 18 November 2026.

Two things decide whether you can get it today. The first is whether your BMI and conditions match an open cohort. The second is whether your GP practice has opted in, because not every practice prescribes yet.

The cohort timeline

Cohort Open from BMI Conditions needed Status
Cohort I 23 June 2025 40 or more At least 4 of 5 Open
Cohort II 23 June 2026 35 to 39.9 At least 4 of 5 Open
Cohort III 2027/28 40 or more 3 Announced, not open
Later cohorts Not dated Lower thresholds Fewer Planned across up to 12 years

BMI thresholds are adjusted downward for people from listed ethnic backgrounds, so the figure your practice applies may be lower than the one in this table.

The five qualifying conditions

The cohorts count conditions, not just BMI. The five are type 2 diabetes, high blood pressure, heart disease, obstructive sleep apnoea and high cholesterol or dyslipidaemia.

This is where most people fall outside the rules. A BMI of 42 on its own does not open Cohort I. You need the BMI and at least four of those five.

To see where you stand against the current thresholds, the NHS Mounjaro guide has a checker that works it out for you.

What might change, and what probably will not

The dates above are plans, not guarantees. Cohort III is announced for 2027/28 and that could move in either direction depending on supply, budgets and how the first two cohorts have gone.

What is unlikely to change soon is the overall shape. A 12-year rollout means most people with a BMI between 30 and 35, or 35 to 40 with fewer than four conditions, are years from NHS access. If you are in that group, planning around a date that has not been announced is a mistake.

Watch the NICE decision on Foundayo on 18 November 2026. A second licensed option changes the supply picture, and supply is one of the things holding the rollout back.

Common questions

Which cohort is open right now?

Cohorts I and II. That means BMI 40 or more with at least 4 of 5 qualifying conditions, or BMI 35 to 39.9 with at least 4.

When will my BMI range be covered?

Only Cohort III has a date, 2027/28, for BMI 40 or more with 3 conditions. Nothing beyond that is dated. The programme is planned to run up to 12 years in total.

Can I get on a waiting list now?

There is no national waiting list for future cohorts. You are assessed when your cohort opens and your practice prescribes.

Does every GP practice prescribe it?

No. Practices opt in, so two people in the same cohort can have different access depending on where they are registered.

Can I get it on the NHS if I am just overweight?

No. No announcement has been made offering NHS weight loss injections to people in the overweight BMI range.
